Sunnyday Carers is committed to providing quality services and respecting your rights. Your right to privacy and confidentiality will be recognized, respected, and protected in all aspects of your contact with us. This statement outlines our ongoing obligations to you in respect to how we manage your Personal Information.

What is Personal Information and Why Do We Collect It?

Personal Information is information or an opinion that identifies an individual. This includes Health Information, relating to physical or mental health or disability.

Examples of Personal Information we collect include, but are not limited to:

  • Contact details of participants and their representatives
  • Emergency contacts and authorized representatives
  • Health status, medical and medication records
  • Service intake, assessment, and review documentation
  • External agency information
  • Feedback, complaints, incident reports
  • Consent for photography and marketing

We collect Personal Information through interviews, emails, telephone calls, our website, publicly available sources, and third parties. We only collect information necessary to:

  • Assess your eligibility for support
  • Deliver safe and responsive support
  • Monitor services provided
  • Meet government reporting obligations

Third Parties

Where possible, we collect information directly from you. However, sometimes we may receive information from third parties (e.g. other disability service providers). We will take reasonable steps to ensure you are informed when this occurs.

Disclosure of Personal Information

Your information may only be disclosed:

  • To address a serious or imminent threat to health or life
  • To agencies with your or your representative’s permission
  • With written consent from a lawful authority
  • When required by law or legislation

Security and Destruction

We store your Personal and Health Information securely and protect it from misuse, loss, and unauthorized access. When no longer required, we destroy or de-identify the data in line with our Privacy and Confidentiality Policy.

Access to Your Information

You may request to access or update your Personal or Health Information. We may require identification to confirm your identity. Requests may be denied if:

  • The request is frivolous or vexatious
  • It impacts another individual's privacy
  • It may interfere with legal investigations
  • It poses a serious threat to life or health
  • It is legally restricted

We aim to respond within 5 working days. No fee is charged for access requests, but administrative costs may apply for copies.
